SARBET-GM

This composition contains betamethasone, gentamycin and miconazole are used together to treat skin infections, it blocks the production of certain chemical which releases these inflammatory and pain mediators. They stimulate the growth of bacteria.

Betamethasone is corticosteroid which is belong to the second-generation and synthetically. It is exits as prodrug. It possesses less affinity glucocorticoid receptor.

Gentamicin is aminoglycoside derivative and has broad-spectrum activity. they only act on the synthesis of proteins. It is useful for both gram-positive and gram-negative bacteria.

Miconazole is azole class of antifungal drugs and is used mainly for fungal infections. It is less absorbed when applied topically.

  • Eczema
  • Dermatitis
  • Keratitis bacterial
  • Meningitis

Mechanism of action:

Betamethasone is converted into its metabolite i.e. 17 monopropionate and translocate into the nucleus and binds to the glucocorticoid response element leads to transcription of gene having anti-inflammatory activity.
Gentamycin binds to the 16S rRNA of 30s subunit and forms toxic misleading proteins.
Miconazole inhibits the enzyme 14-alpha demethylase into ergosterol from lanosterol.

Pharmacokinetics:

Beclomethasone-
It is absorbed orally
It is 96% bound to plasma proteins
It undergoes hydrolysis
It is excreted via feces

Miconazole-
It is absorbed orally
It mainly bound to serum proteins
It is metabolized in the liver
It is excreted via feces and urine
